What should you know about fulminant pneumonia?

Pneumonia inflames the air sacs and surrounding tissue in the lungs. In a fulminant form, the illness can worsen quickly, limiting oxygen exchange and placing strain on the heart and other organs. It may follow a bacterial or viral infection, including influenza or COVID-19. Severe infection can also trigger sepsis, a dangerous whole-body response.

Possible features include fever, chills, cough, phlegm, chest discomfort, fast breathing, marked weakness, or breathlessness. A cough with phlegm is a common presentation of pneumonia, but these symptoms do not identify the cause on their own.

If a clinician suspects severe pneumonia, go for assessment without delay. The team may check breathing, oxygen levels, blood pressure, temperature, and mental alertness, then use chest imaging and laboratory tests to guide treatment. Care can include oxygen, fluids, and medicines directed at the suspected infection; hospital monitoring may be needed.

While arranging care, keep the person sitting upright if breathing is difficult, avoid exertion, and take only medicines recommended by a clinician. Do not delay assessment while trying to treat severe breathlessness or confusion at home.

Call emergency services immediately for severe difficulty breathing, blue or grey lips, new confusion, fainting, inability to stay awake, or rapidly worsening weakness. Urgent assessment is also needed when fever and cough are accompanied by fast breathing, chest pain, very little urine, or a person who cannot keep fluids down. Children, older adults, pregnant people, and those with heart, lung, or immune conditions may deteriorate with less warning.

Ask what may have caused the infection, whether oxygen monitoring or admission is needed, which tests have been arranged, how treatment will be reviewed, and which warning signs should prompt immediate return. You can also ask whether close contacts need advice if influenza or COVID-19 is suspected.
