Fluctuating Hearing Loss in Meniere's Disease: What Should You Know?

Fluctuating hearing loss can occur in Meniere's disease, often changing with episodes of ear pressure, ringing or vertigo. A new, sudden or one-sided change still needs assessment because other ear and neurological problems can cause similar symptoms.

Meniere's disease affects the inner ear and may produce attacks of spinning vertigo, ringing in the ears, pressure or fullness, and changing hearing. Hearing may feel muffled during an episode and clearer later; over time, some people notice a lasting change. The pattern, affected ear, attack timing and associated symptoms help distinguish it from wax, infection, medication effects or other causes.

Keep a diary of hearing changes, ear pressure, ringing, dizziness, vertigo, triggers and medicines. During a vertigo spell, sit or lie somewhere safe, avoid driving and protect yourself from falls. Arrange a GP consultation or hearing test for a new, recurrent or troublesome change. An audiology or ear specialist assessment may be recommended, and treatment depends on the findings rather than the hearing sensation alone.

Seek urgent medical help for sudden hearing loss, new facial weakness, trouble speaking, severe new headache, fainting, weakness on one side, or vertigo with inability to walk safely. Prompt assessment is also sensible for severe ear pain, discharge, fever or symptoms after a head injury. Do not assume a sudden change is simply another Meniere's episode.
