Hearing loss: causes, warning signs and care

Hearing loss is a reduction or distortion in hearing that should be assessed promptly when it starts suddenly or affects one ear.

Sounds may seem quieter, muffled, distant, or unclear, and conversation can be harder to follow when there is background noise. You may turn up the volume, ask people to repeat themselves, or notice a blocked-ear sensation. Ringing, dizziness, ear pain, discharge, itching, fever, or pressure are useful details because they can point toward different parts of the ear.

Seek same-day medical advice for sudden hearing loss, a rapid change in hearing, or new hearing loss in one ear. Emergency assessment is appropriate when hearing change comes with severe dizziness, facial weakness, a severe headache, confusion, difficulty speaking, or weakness in an arm or leg. Ear pain with swelling behind the ear, high fever, blood, or pus also needs prompt care.

Hearing can be affected by earwax buildup, Meniere's disease, a middle ear infection, or swimmer's ear. These causes are listed alphabetically, not in order of likelihood. Earwax may block the ear canal; outer-ear inflammation may cause itch, pain, and discharge; middle-ear infection can cause pressure and pain; and Meniere's disease can involve episodes of vertigo, ringing, and a feeling of fullness. A clinician can check whether another cause needs consideration.

The appointment usually begins with when the change started, whether one or both ears are affected, recent colds, swimming, noise exposure, medicines, dizziness, and ear discharge. The clinician can examine the outer ear and eardrum, then arrange hearing testing when needed. Further testing may be considered if the hearing change is sudden, one-sided, recurring, or linked with balance or nerve symptoms.

Treatment is based on the cause and the part of the ear involved. A clinician may advise safe earwax removal, prescribe treatment for infection or inflammation, review medicines, or refer for hearing and balance assessment. Do not push cotton buds, keys, hairpins, or other objects into the ear canal; they can compact wax or damage the skin. Ear wax removal is performed using a method chosen after the ear has been examined.

Use hearing protection around loud equipment or music, give your ears quiet breaks, and follow workplace safety guidance. Keep the outer ear dry after swimming when you are prone to irritation, but avoid probing inside the canal. Discuss recurring wax, infections, or balance symptoms with a clinician rather than using ear drops or home devices that may not suit your ear.

Tell the clinician about ringing, popping, pressure, pain, itch, discharge, fever, vertigo, nausea, facial weakness, or a blocked sensation. Changes in speech understanding, especially in noisy places, are also relevant even when everyday sounds still seem audible.

Untreated hearing difficulty can make communication, work, study, driving, and social situations more tiring. Follow-up can confirm whether hearing has returned, identify a recurring ear problem, and provide support if the change continues. Return sooner if pain, discharge, dizziness, or a further drop in hearing develops.
