Flail Chest on X-Ray: What the Image Can Show

Flail chest is a serious chest injury in which a section of the rib cage is detached from the surrounding chest wall because of multiple rib fractures. A chest X-ray can help identify rib fractures, air or blood around a lung, and signs of lung injury. Rib fractures can be difficult to see on a plain image, so a normal-looking X-ray does not rule out significant injury. Clinicians also assess chest movement, pain, oxygen level and breathing effort.

Do not try to strap or tightly bind the chest, because restricting movement can make breathing harder. Keep the injured person still and supported, and follow emergency instructions. In hospital, treatment may include oxygen, pain relief that allows deeper breaths, monitoring, and support for coughing and clearing lung secretions. Additional imaging may be considered when the injury or its complications are not clear. Recovery plans depend on the fractures, breathing function and other injuries.

Call emergency services after major chest trauma if breathing is difficult, chest movement looks uneven, pain is severe, the person becomes drowsy or confused, or the lips look blue or grey. Worsening breathlessness, fever, or a cough after the injury also needs prompt medical review because lung complications such as pneumonia can develop.

Ask which findings were seen, whether another scan is needed, how to control pain while breathing deeply, and which symptoms should trigger an immediate return to hospital.
