First period after a cone biopsy

A cone biopsy removes a cone-shaped piece of cervical tissue. During healing, light bleeding or watery discharge may occur, and the next menstrual bleed can be difficult to distinguish from healing-related spotting. Timing and flow may temporarily differ from your usual cycle. The procedure is commonly used to investigate or treat Cervical Dysplasia; the pathology result, rather than the appearance of the first period, guides follow-up. Cone biopsy is not itself a diagnosis of Cervical Cancer.

Follow the discharge instructions from your procedure team. Use sanitary pads so you can observe the amount and colour of blood, and avoid putting anything in the vagina, including tampons and menstrual cups, until you have been told healing is complete. Avoid strenuous activity if it increases bleeding. Keep your follow-up appointment so the pathology result and future cervical screening plan can be explained. Tell the team if you use blood-thinning medicine or could be pregnant.

Contact your procedure team urgently for bleeding that is becoming heavy, large clots, severe or worsening pelvic pain, fever, chills, or discharge with an unpleasant smell. Seek emergency help if you feel faint, weak, confused, or short of breath. A small amount of spotting can be expected after treatment, but a sudden change deserves advice rather than waiting for the next appointment.

Ask when internal products, sex, swimming, and exercise are safe for you. Clarify whether your bleeding is within the expected healing pattern, when pathology results will be available, and which symptoms should prompt an out-of-hours call. Ask whether your follow-up involves colposcopy or another cervical check.
