The short answer

The medical term is calcium pyrophosphate deposition disease, or CPPD. It can cause an attack that resembles gout, with pain, warmth, tenderness, and swelling. A knee is a common site, although the wrist, ankle, shoulder, or another joint can also be involved. Because several joint conditions look alike, a clinician may need to examine the joint fluid or use imaging to identify the crystal type.

What false gout means

In CPPD, calcium pyrophosphate crystals collect in cartilage or around a joint. The immune response to these crystals can produce a rapid flare of inflammation. This is different from gout, which is driven by urate crystals; the symptoms alone cannot reliably separate the two.

A flare may begin abruptly and make movement difficult. Some people have repeated attacks, while others develop a more persistent pattern of joint discomfort or stiffness. Age-related cartilage changes can contribute, and a clinician may consider other health factors when CPPD appears earlier than expected. Kidney disease and obesity are relevant health conditions to discuss, but neither one proves that a painful joint is gout or false gout.

What to do during a suspected flare

Arrange medical assessment for a new, hot, swollen, or very painful joint, especially if the diagnosis has not been confirmed. Rest the affected joint, keep it comfortably raised, and use a wrapped cold pack for short periods if that feels soothing. Avoid putting extra strain on the joint while it is acutely inflamed.

Treatment depends on your health history, the affected joint, and whether infection has been excluded. A clinician may recommend an anti-inflammatory medicine, a steroid treatment, or another option. Do not start leftover gout medicine or an over-the-counter anti-inflammatory without checking that it is suitable, particularly if you have kidney disease, stomach ulcers, heart problems, take blood thinners, or are pregnant. Once the flare settles, follow-up can help address recurrent attacks and mobility.

When to seek care urgently

Seek urgent medical care for a suddenly swollen and intensely painful joint with fever, chills, feeling faint, marked redness, or rapidly worsening symptoms. A joint infection can resemble a crystal flare and needs prompt treatment. Urgent assessment is also sensible after a significant injury, when you cannot bear weight, or when the joint looks deformed.

Contact a clinician soon if attacks keep returning, pain does not improve, several joints become swollen, or stiffness is limiting everyday activities. Do not assume that a previous diagnosis explains every future flare; a different problem can affect the same joint.
