Possible causes

During breathing out, narrowed airways can create a high-pitched musical noise. Asthma often presents with variable wheeze and may come with cough, chest tightness, or episodes after exercise or exposure to a trigger. Acute bronchitis can inflame the tubes after an infection. Allergic rhinitis may contribute through nasal inflammation and drainage. COPD can cause ongoing airflow limitation, particularly in people with a history of smoking or other inhaled irritants. Bronchiolitis affects small airways in young children. Sudden wheeze with swelling, hives, vomiting, or throat tightness can signal anaphylaxis and needs emergency care. The sound itself cannot identify the cause.

How to respond

Move away from smoke, dust, fumes, or a known allergen and sit upright if breathing feels tight. Record the timing, exposure, activity, fever, cough, and any previous lung diagnosis. If a clinician has prescribed an inhaler, follow the personal action plan and instructions. A clinician may listen to the chest, check oxygen levels, and decide whether breathing tests or other assessment is needed. Nebuliser treatment is used in selected situations and should be guided by a professional.

Warning signs

Call emergency services for severe breathlessness, blue or grey lips, faintness, confusion, inability to speak in full phrases, or fast deterioration. Treat sudden wheeze with facial or tongue swelling, throat tightness, or widespread hives as an emergency. Seek prompt medical advice for a first episode, recurrent symptoms, chest pain, coughing blood, or wheeze that is interfering with sleep or normal activity.
