Ectopic pregnancy and periods

An ectopic pregnancy may cause vaginal bleeding because pregnancy-related tissue changes the lining and nearby blood vessels. The bleeding may be lighter, heavier, earlier or different in colour from your usual period, although appearance alone cannot identify its cause. A period-like bleed does not rule out pregnancy. Ectopic pregnancy presents with bleeding in some people and may also cause pelvic pain, often on one side. Other explanations include an early miscarriage, cervical bleeding or changes in the menstrual cycle. Only an assessment can distinguish these possibilities.

Take a pregnancy test if pregnancy is possible, even if bleeding has started. Contact a clinician promptly if the result is positive, unclear or symptoms continue. They may repeat blood testing and arrange a Transvaginal Ultrasound because a very early pregnancy may not be visible on the first scan. Keep track of pain, bleeding and any dizziness, and tell the clinician about prior Pelvic Inflammatory Disease or surgery involving the tubes.

Seek emergency help for severe or increasing pelvic pain, fainting, dizziness, shoulder-tip pain, unusual weakness or heavy vaginal bleeding. These symptoms can signal internal bleeding and require immediate care, regardless of whether the bleeding initially looked like a period.

Ask whether the bleeding pattern could be related to pregnancy, whether your pregnancy hormone level needs repeating, when the ultrasound should be repeated, and which warning signs mean you should return urgently.
