Ectopic pregnancy: first symptoms

An ectopic pregnancy develops outside the main cavity of the uterus, often in a fallopian tube. Early symptoms may resemble a normal early pregnancy or a miscarriage, so symptoms alone cannot confirm the cause. Pain may sit low in the abdomen or pelvis and can be stronger on one side. Spotting or bleeding may occur. Some people have no clear symptoms at first. A pregnancy test can be positive, but the timing and level of pregnancy hormone may affect results. If pregnancy is possible and you have pelvic pain or unusual bleeding, contact a gynaecology service promptly. Note when the pain began, where it is located, whether it is worsening, and the amount and colour of bleeding. A clinician may arrange blood tests and a [Transvaginal Ultrasound](/treatments/diagnostics-and-imaging/transvaginal-ultrasound/) to locate the pregnancy and assess your pelvis. Do not wait for symptoms to become dramatic before seeking advice.

Go to an emergency department immediately for sudden or severe abdominal pain, fainting, marked dizziness, weakness, shoulder-tip pain, or heavy bleeding. These signs can indicate internal bleeding and need rapid assessment. Severe symptoms matter even if a home pregnancy test is unclear or negative. Do not drive yourself if you feel faint.

Ask whether your symptoms require repeat pregnancy-hormone testing, when imaging should be repeated, and which changes should trigger emergency care. If treatment is needed, ask how medicines or surgery affect follow-up and future pregnancy plans.
