Early ectopic pregnancy

An ectopic pregnancy is a pregnancy growing outside the uterine cavity. Early signs may include delayed menstruation, pelvic pain, vaginal bleeding, discomfort on one side, or pain felt at the shoulder. Symptoms can overlap with normal early pregnancy and other conditions, so their presence does not identify the cause. Pelvic pain is a symptom that needs context, especially alongside a positive pregnancy test.

A clinician may use serial pregnancy hormone tests and a transvaginal ultrasound. An early scan may not yet show a clear pregnancy location, so a planned review can be needed. The assessment also considers previous ectopic pregnancy, tubal surgery, fertility treatment and pelvic inflammatory disease.

After a positive pregnancy test, report pain or bleeding to your maternity or gynaecology team. Record the amount and pattern of bleeding, the location and severity of pain, and any dizziness. Do not wait for a routine appointment if symptoms are worsening. Keep follow-up appointments even when discomfort settles, because the location of a very early pregnancy may still need confirmation.

Go to an emergency department for severe or rapidly worsening pain, fainting, collapse, unusual weakness, heavy bleeding, shoulder-tip pain or breathing difficulty. Internal bleeding can develop without a long warning period. If testing confirms an ectopic pregnancy, clinicians may discuss observation, medicine or surgery. A salpingectomy removes an affected fallopian tube when surgery is needed, but the appropriate choice depends on individual findings.
