What should you know about dark red sputum?
Updated

Short answer
Dark red sputum may contain blood from irritated airways, infection, or bleeding in the breathing passages.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Record
- Amount, color, frequency, fever, chest pain, and breathlessness.
- Do not self-treat
- Do not use leftover antibiotics or suppress worsening breathing symptoms.
What dark sputum can indicate
Rust-colored or dark red phlegm can appear after forceful coughing irritates the throat or bronchial tubes. Acute bronchitis and pneumonia may produce sputum with blood alongside cough, fever, chest discomfort, or breathlessness. Bronchiectasis can cause repeated phlegm and bleeding, while COPD may make airway symptoms more persistent. Blood from a nosebleed or irritated sinuses can drain backward and be coughed out, so the source is not always the lungs. The amount, frequency, and accompanying breathing symptoms are more useful than color alone.
What to do next
Note the color, amount, texture, and whether the material came after coughing or throat clearing. Save a photograph if it is safe and practical, and record fever, chest pain, breathlessness, wheezing, smoking history, lung disease, and medicines that affect clotting. Rest and drink fluids if you can. Do not start leftover antibiotics; antibiotic treatment is useful only for selected infections and requires clinical judgment. Arrange medical review for unexplained, recurrent, or increasing blood-stained sputum.
When to seek care
Call emergency services for more than a small streak of blood, active or repeated bleeding, severe breathlessness, blue lips, chest pain, faintness, confusion, or a very fast heartbeat. Seek same-day assessment for fever with worsening cough, new breathlessness, or blood-stained sputum in someone with COPD or known lung disease. A persistent change in phlegm also warrants review even when breathing feels stable.
Questions for your doctor
Could the blood be from my nose, throat, or lungs? Do I need a chest examination, imaging, or a sputum test? Would antibiotic treatment help, and which symptoms should trigger emergency care?

Questions people ask
Can hard coughing make sputum dark red?
Yes. Forceful coughing can irritate small blood vessels, but repeated or unexplained blood-stained sputum still needs medical review.
Could the blood come from my nose?
Yes. Nasal or sinus bleeding can drain into the throat and later appear when you cough or clear phlegm.
