What should you know about dark red sputum?

Rust-colored or dark red phlegm can appear after forceful coughing irritates the throat or bronchial tubes. Acute bronchitis and pneumonia may produce sputum with blood alongside cough, fever, chest discomfort, or breathlessness. Bronchiectasis can cause repeated phlegm and bleeding, while COPD may make airway symptoms more persistent. Blood from a nosebleed or irritated sinuses can drain backward and be coughed out, so the source is not always the lungs. The amount, frequency, and accompanying breathing symptoms are more useful than color alone.

Note the color, amount, texture, and whether the material came after coughing or throat clearing. Save a photograph if it is safe and practical, and record fever, chest pain, breathlessness, wheezing, smoking history, lung disease, and medicines that affect clotting. Rest and drink fluids if you can. Do not start leftover antibiotics; antibiotic treatment is useful only for selected infections and requires clinical judgment. Arrange medical review for unexplained, recurrent, or increasing blood-stained sputum.

Call emergency services for more than a small streak of blood, active or repeated bleeding, severe breathlessness, blue lips, chest pain, faintness, confusion, or a very fast heartbeat. Seek same-day assessment for fever with worsening cough, new breathlessness, or blood-stained sputum in someone with COPD or known lung disease. A persistent change in phlegm also warrants review even when breathing feels stable.
