What should you know about dark pigmentation around neck?

Acanthosis nigricans often appears in skin folds such as the neck and may look darker, thicker and velvety. It can be associated with insulin resistance, some hormonal conditions, certain medicines or, less commonly, another underlying illness. The skin change itself does not establish the cause, and rubbing or scrubbing it hard can worsen irritation.

Other possibilities include friction from collars or jewellery, contact irritation from hair or skincare products, eczema, a fungal infection or pigmentation left after inflammation. A clinician will consider whether the area is itchy, scaly, sore, spreading or present elsewhere, and may review medicines and general health concerns.

Arrange a dermatology or primary-care assessment if the change persists, becomes thicker or is not explained by a clear recent irritation. Take note of when it began, products that touch the neck, any itching or scaling, and whether similar changes are present in the armpits or groin. Do not use bleaching creams, strong acids or steroid creams without advice.

Treatment depends on the finding. Managing an underlying metabolic or hormonal issue may help acanthosis nigricans, while gentle cleansing, moisturising and reducing friction can support irritated skin. After assessment, a clinician may discuss topical retinoids, a chemical peel or laser toning; these treatments can irritate or temporarily darken some skin types and should be selected and monitored professionally.

Book a medical review soon if the pigmentation appears suddenly, spreads quickly, becomes painful, cracks, bleeds or is accompanied by marked itching, scaling, weight change, unusual thirst or frequent urination. Seek urgent care for facial or neck swelling, trouble breathing, fever with rapidly worsening redness, or a severe allergic reaction.

A new or changing patch should be examined rather than treated only for colour. Prompt assessment helps distinguish a fold-related condition from infection, dermatitis or a sign of a wider health problem.
