Dandruff with a bald spot: what to know

Dandruff may cause itch and scratching, but a smooth or sharply outlined bald patch is not explained by ordinary flaking alone. Alopecia areata can present as smooth patches, while scalp ringworm can cause scale, itch, broken hairs, or tender areas. Hair pulling, friction, and other scalp inflammation can also produce localized loss. Note whether the patch is smooth, scaly, swollen, tender, or dotted with broken hairs; these details help guide examination.

Avoid scratching, sharing combs or hats, and applying strong chemicals to the patch. Wash gently and do not cover worsening inflammation with tight headwear. Take a clear photograph and arrange a dermatology assessment, particularly when the spot is enlarging. Treatment depends on the cause; a hair transplant is not a first step while an active inflammatory or infectious process remains possible.

Arrange prompt medical advice for a new bald patch, especially with scaling, broken hairs, pus, swelling, pain, fever, or tender glands. Seek quicker assessment if a child has a scaly patch or if close contacts develop similar symptoms, because scalp infections can spread. Sudden additional patches also warrant review.

Ask whether the patch looks more consistent with alopecia areata, scalp ringworm, irritation, or another condition. Ask whether testing is needed, what products to avoid, and how to monitor regrowth. Mention contact with affected people, pets, new hair products, and any recent illness.
