Cryptococcus pneumonia: what to know

Cryptococcus is a fungus found in the environment. Infection can begin when fungal particles are inhaled and reach the lungs. Some people have few symptoms, while others develop a cough, fever, chills, chest discomfort, tiredness, or coughing up phlegm. These symptoms overlap with other causes of pneumonia, including viral infections, so symptoms alone cannot identify the organism.

The risk of severe or spreading infection is higher when the immune system is weakened. Infection can sometimes affect areas beyond the lungs, including the brain, which is why clinicians may ask about headache, confusion, unusual sleepiness, or changes in behaviour. Testing may involve imaging and laboratory examination of respiratory or other samples.

Follow the prescribed antifungal course and attend planned reviews. Treatment choice and duration depend on the severity of lung disease, immune status, test results, and whether infection has spread. Do not substitute leftover antibiotics or stop treatment because breathing improves. Tell the care team about every medicine and supplement you take, since antifungal medicines can require monitoring and may interact with other medicines.

Rest, drink enough fluid if your clinician has not restricted it, and avoid smoking or inhaled irritants. Keep a record of fever, breathing symptoms, chest pain, and alertness. People with weakened immunity should contact their treating team early when symptoms change, rather than waiting for a routine appointment.

Get emergency help for severe or worsening breathlessness, blue or grey lips, chest pain, fainting, new confusion, difficulty staying awake, or coughing blood. Urgent assessment is also needed for a high or persistent fever, rapidly worsening weakness, or signs of a serious infection such as cold clammy skin and very fast breathing. A new severe headache, neck stiffness, vomiting, or light sensitivity during cryptococcal infection needs immediate medical attention because it may indicate spread beyond the lungs.
