Short answer

Cramping between ovulation and a period may reflect normal cycle changes, but persistent, severe, or unusual pain needs assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Possible explanations

Ovulation can irritate the pelvic lining briefly, while the second half of the cycle brings hormonal changes before menstruation. Pain may be felt on one side, in the centre, or across the pelvis. Other explanations include endometriosis, adenomyosis, fibroids, an ovarian endometrioma, or pelvic inflammatory disease. The timing of pain can offer a clue, but it does not identify a condition by itself. New pain after a missed period also warrants a pregnancy check.

Ways to track and ease it

Keep a cycle diary with the first day of bleeding, suspected ovulation, pain position, severity, duration, discharge, bowel or bladder symptoms, and medicines taken. Gentle movement, rest, warmth over the lower abdomen, and hydration may help some people. A pharmacist or clinician can help choose suitable pain relief, especially if you might be pregnant or have stomach, kidney, liver, or bleeding problems. Ask about options such as the Combined Pill or Hormonal Coil if cramps repeatedly disrupt daily life.

When to seek care

Arrange medical review if pain is recurring, worsening, lasts beyond the expected cycle, or affects work, sleep, sex, bowel movements, or urination. Seek urgent care for sudden severe pain, fainting, fever, vomiting, a swollen abdomen, or pain with a positive pregnancy test. Prompt assessment is also sensible with unusual discharge or pain during sex.

Useful questions

Ask whether your pattern suggests ovulation pain or painful periods, whether a pelvic examination or ultrasound is appropriate, and which treatments fit your pregnancy plans. Bring your cycle diary and mention prior pelvic infections, surgery, or difficulty conceiving.
