Cough after croup

A cough may remain after croup as the upper airway settles, but stridor at rest or increased breathing effort requires urgent assessment. A barking cough can be more noticeable at night or with crying. Monitor the child between coughs: comfortable breathing, drinking and normal responsiveness are useful signs.

Croup causes swelling around the voice box and windpipe. Even after the bark eases, irritation can provoke coughing. A cough that continues without noisy breathing may be part of recovery, but recurrent episodes can also be associated with common colds, allergic rhinitis, asthma or acid reflux. The pattern and breathing sounds help guide assessment.

Keep the child calm, because crying can make upper-airway noise and coughing worse. Offer fluids, allow rest and keep the air free of smoke. Hold the child upright if that is comfortable. Avoid steam burns and do not use cough medicines unless a clinician recommends them for that child.

Call emergency services for severe breathing difficulty, blue or grey colour, pauses in breathing, exhaustion, drooling with difficulty swallowing, or stridor that remains when the child is calm. Seek prompt medical advice if the cough is worsening, drinking falls, fever returns, or noisy breathing comes back. A clinician can distinguish recovery from conditions such as bronchiolitis or chronic cough.

Ask what breathing sound should prompt urgent action, whether the episode needs follow-up, and whether repeated barking coughs suggest another trigger. Share when symptoms occur and what happens during sleep or activity.
