What should you know about complex migraine?

People may experience a throbbing or pressure-like headache alongside visual disturbance, tingling, numbness, dizziness, vertigo, trouble finding words, light sensitivity, nausea or vomiting. Facial pressure can occur, although it does not by itself prove a sinus problem. Aura symptoms often develop gradually and resolve, but their timing and pattern matter.

The term is used inconsistently, so a clinician may describe the episode as migraine with aura or another migraine subtype. A new neurological symptom should not automatically be attributed to migraine, particularly when it begins abruptly, differs from previous attacks, or occurs with fainting, severe weakness or confusion.

Rest in a quiet, dim room, sip fluids if able, and use the treatment plan prescribed for you as early as directed. A regular sleep pattern, consistent meals and a diary of headaches, triggers, aura and medicines can help identify patterns. Frequent use of acute pain medicine can itself make headaches harder to control, so review the plan if attacks or rescue medicine use are increasing.

A clinician can check blood pressure, neurological function, medicines and other possible causes. Preventive treatment may be considered when attacks are disruptive or recurring; the choice depends on symptoms, other conditions, pregnancy plans and treatment response.

Seek emergency help for a sudden severe headache, new one-sided weakness, facial drooping, difficulty speaking, loss of vision, seizure, fainting, confusion, fever with a stiff neck, or symptoms after a head injury. Urgent review is also appropriate when an attack is new, unusually prolonged, or clearly different from your usual migraine.

Ask whether your symptoms fit migraine with aura, whether another cause needs exclusion, which rescue treatment is suitable, and how often preventive treatment should be reviewed. Bring your headache diary and a list of medicines, including non-prescription products.
