What should you know about collision dyspareunia?

Short answer

Collision dyspareunia is deep pelvic pain during penetration, often triggered when thrusting puts pressure on the cervix or other sensitive pelvic structures.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Pain location
Deep in the pelvis or lower abdomen rather than only at the vaginal entrance
Common trigger
Deep penetration, certain angles, or thrusting
Useful detail
Whether pain changes with position, menstruation, urination, or bowel movements

Direct answer

The term describes a pain pattern rather than a diagnosis. Pain may depend on depth, angle, position, the stage of the menstrual cycle, or whether pelvic tissues are already tender.

What the pain pattern may mean

Pain felt at the vaginal entrance differs from collision pain, which is usually felt deeper in the pelvis or lower abdomen. Noting the exact location helps a clinician distinguish surface irritation from pressure on deeper structures.

Deep pain can occur when the cervix, pelvic floor muscles, ovaries, uterus, bladder, bowel, or inflamed pelvic tissue becomes sensitive to movement or pressure. Endometriosis and pelvic inflammatory disease can present with deep pain during sex, but symptoms alone cannot identify the cause.

A clinician may ask about periods, discharge, bleeding, urinary or bowel symptoms, pregnancy possibility, previous pelvic procedures, and whether the discomfort happens every time or only in certain positions. An examination or imaging may be offered according to that history.

What you can do now

Stop or change the activity when pain starts; pushing through can increase muscle guarding. Positions that let you control depth and angle, slower movement, and avoiding deep thrusting may reduce pressure. Lubricant can help if friction is also present, although it may not relieve pain arising deeper in the pelvis.

Keep a brief symptom record showing where the pain is felt, which movements trigger it, its relationship to menstruation, and any bleeding, discharge, fever, urinary symptoms, or bowel symptoms. Arrange a gynaecology assessment if the pain recurs, limits intimacy, or is becoming more intense. Treatment depends on the cause and may include pelvic floor therapy, medication, hormonal management, infection treatment, or a procedure.

When to seek care

Seek urgent medical care for severe or rapidly worsening pelvic pain, fainting, dizziness, shoulder-tip pain, heavy bleeding, or pain with a possible pregnancy. These features need prompt assessment, including for pregnancy-related emergencies.

Prompt assessment is also appropriate when deep pain occurs with fever, vomiting, foul-smelling discharge, new bleeding after sex, or marked lower-abdominal tenderness. These symptoms can accompany infection or another condition needing timely treatment.

Questions for your appointment

You might ask which structures could be tender, whether a pelvic examination or ultrasound would clarify the cause, and whether pelvic floor muscle tension may be contributing. Also ask what changes are safe while the cause is being assessed and which new symptoms should prompt urgent care.

Questions people ask

Is collision dyspareunia a medical diagnosis?

No. It names a pattern of deep pain caused by pressure or movement during penetration. A clinical assessment is needed to work out whether pelvic floor tension, endometriosis, infection, an ovarian problem, or another cause is involved.

Can changing sexual position help?

It may. Positions that give the person with pain control over depth and angle can reduce contact with tender areas. Recurrent pain still deserves assessment even when a position change provides relief.

Does deep pain during sex mean endometriosis?

Endometriosis can present with deep pain during sex, especially alongside painful periods or other pelvic symptoms, but several conditions can cause the same pattern. The timing, examination findings, and any imaging guide the diagnosis.
