Chronic tophaceous gout: what it means and how it is managed

The short answer
Tophi can develop around fingers, toes, feet, ankles, knees, elbows, or ears. They may be visible beneath the skin, restrict movement, press on nearby structures, or occasionally break through the skin and release pale, chalk-like material.
Why tophi form
Gout occurs when monosodium urate crystals collect in joints and tissues. Tophi indicate a substantial, long-standing crystal burden, even if acute flares are currently infrequent. Untreated deposits can erode bone and cartilage, alter joint shape, and make everyday movement harder.
Diagnosis is often based on the characteristic lumps and history of gout, supported by blood urate testing and sometimes imaging. Fluid or material from a joint or lump may be examined for crystals when the diagnosis is uncertain or infection must be excluded. Kidney function influences medicine selection and dosing but does not make effective management impossible.
Long-term control and flare care
Treatment usually combines medicine for an active flare with ongoing urate-lowering therapy. The long-term medicine is adjusted using blood urate results so existing crystals can gradually dissolve and further deposits are less likely. Take it consistently unless the prescriber tells you to change it; stopping and restarting can destabilise control.
Review kidney health, all medicines, alcohol intake, hydration, and eating patterns with the clinician. Lifestyle changes can support treatment but do not replace urate-lowering medicine when tophi are present. During a flare, protect the joint, use a wrapped cold pack if comfortable, and follow the prescribed anti-inflammatory plan. Surgery is reserved for selected deposits causing skin breakdown, nerve pressure, infection concern, major deformity, or severe loss of function.
When swelling or a tophus needs urgent review
Seek same-day medical assessment for a hot, very painful swollen joint with fever, chills, or marked illness. Joint infection can resemble a gout flare and needs prompt exclusion. Also get urgent help for rapidly spreading redness, pus, darkening skin, or an open wound over a tophus.
Contact your clinician soon if a lump grows, starts draining, causes numbness, limits footwear or hand use, or if flares continue despite taking treatment as prescribed.

Can tophi shrink?
They can become smaller when blood urate is kept at the treatment target over time. Large deposits may take considerable time to change.
Should urate-lowering medicine be stopped during a flare?
Do not stop a regular urate-lowering medicine solely because a flare starts unless your prescriber directs you to. The flare itself is treated with a separate plan.
Is every painful swollen joint a gout flare?
No. Injury and joint infection can produce similar symptoms. Fever, severe illness, an unusual pattern, or a wound near the joint warrants prompt assessment.
