Chocolate Cyst Rupture: Symptoms and Next Steps

A chocolate cyst, also called an ovarian endometrioma, contains old blood linked to endometriosis. If its wall breaks, the contents can irritate the lining of the pelvis. Pain may begin suddenly on one side or across the lower abdomen, and movement may make it worse. Nausea, bloating, light bleeding, or tenderness can occur, but symptoms vary.

Sudden pelvic pain has other possible causes, including ovarian twisting, appendicitis, pelvic infection, or a pregnancy outside the uterus. A symptom cannot confirm that a cyst has ruptured. A clinician may use an examination and imaging, alongside a pregnancy test when relevant, to work out what is happening.

Stop strenuous activity and arrange medical advice promptly, particularly if the pain is new or does not ease. Note when it started, where it is located, whether it is worsening, and whether you have bleeding, fever, vomiting, dizziness, or a chance of pregnancy. Do not drive if you feel faint or impaired.

A clinician may recommend observation, pain relief, repeat assessment, or surgery depending on bleeding, pain, examination findings, and the cyst’s appearance. Treatment for the underlying endometriosis may be discussed after the immediate concern is settled. Existing painful periods or pain during sex are useful details to mention.

Go to an emergency service for severe or rapidly worsening pelvic pain, fainting, marked dizziness, a fast heartbeat, repeated vomiting, fever, a swollen or rigid abdomen, or heavy vaginal bleeding. Seek urgent assessment for pelvic pain with a positive or possible pregnancy test. These warning signs can reflect bleeding, infection, twisting, or another condition that needs rapid treatment.

Ask whether the examination or scan suggests rupture, how bleeding will be checked, which pain relief is suitable, and what symptoms should bring you back urgently. Ask how an endometrioma may affect fertility plans, whether laparoscopic ovarian cystectomy is appropriate, and whether medical treatment could help prevent recurring endometriosis symptoms.
