Chemotherapy for leukemia

Unlike treatment aimed at one solid tumour, leukemia therapy must reach abnormal blood-forming cells in blood and bone marrow. Some medicines are swallowed and others are given into a vein; selected plans also deliver treatment into the fluid around the brain and spinal cord.

Acute leukemias grow quickly and often need treatment started promptly, commonly through several planned phases. Chronic leukemias can behave differently; some are monitored before therapy, and some are treated mainly with targeted medicines rather than traditional chemotherapy.

Laboratory examination of blood and bone marrow identifies the cell type and molecular features that guide treatment. Age, general fitness, infections, organ function, previous therapy and the treatment goal also affect which drugs and intensity are suitable.

Ask whether treatment is inpatient or outpatient and arrange practical support for appointments or admission. Keep the emergency number and temperature instructions with you. Take medicines exactly as prescribed, including preventive medicines, and check before using painkillers, supplements, herbal remedies or vaccines.

Blood counts may fall because leukemia and chemotherapy both affect marrow. Follow advice on infection precautions, mouth care, food handling and activity. Note fever, bruising, bleeding, dizziness, breathlessness, bowel changes or new pain and report them according to the team’s instructions.

Phone the hematology or oncology team immediately for a temperature at or above their stated threshold, chills, confusion, sudden deterioration, new cough, breathing difficulty, persistent vomiting or diarrhoea, or pain when passing urine. Infection during treatment can require urgent testing and medicine.

Get urgent help for bleeding that does not stop, vomit with blood, black stools, severe headache, fainting, chest pain or rapidly worsening breathlessness. Do not wait for a scheduled blood test when these warning signs appear.

Request the full subtype name and ask which test results shaped the regimen. Discuss the purpose and sequence of treatment phases, whether targeted therapy or transplant may be considered, how remission is assessed, possible fertility effects, and whom to call overnight or on weekends.
