Cervical dysplasia stages and what they mean

CIN means cervical intraepithelial neoplasia. These are abnormal cells confined to the surface lining of the cervix; CIN is not cervical cancer. A higher grade means more of that lining contains abnormal cells and usually leads to closer management.

A cervical screening sample examines shed cells and may describe low-grade or high-grade changes. An HPV test checks for types of human papillomavirus linked with cervical cell changes. CIN is diagnosed and graded by examining a tissue sample taken during colposcopy, so the words on a screening report and a biopsy report may differ.

The grade is only one part of the decision. Age, pregnancy, the visibility and size of the affected area, previous results, immune health, and whether the whole area was assessed can influence the follow-up plan.

Read the result together with the recommended action and appointment date. Lower-grade change may be monitored with repeat testing because it can return to normal. Higher-grade change is more likely to be removed, often with a loop excision; a cone biopsy may be considered when a larger or deeper specimen is needed.

Attend follow-up even when you feel well because dysplasia commonly causes no symptoms. If pregnancy or future pregnancy is relevant, discuss how the timing and extent of treatment could affect cervical care. Tell the clinic about previous cervical procedures and bring earlier reports if they were issued elsewhere.

Do not wait for a routine screening appointment if you develop bleeding after sex, bleeding between periods, bleeding after menopause, persistent unusual discharge, or pelvic pain. Cervical dysplasia often has no symptoms, and these symptoms have several possible causes, so they need their own assessment rather than being used to guess a CIN grade.

Seek urgent care for heavy bleeding, fainting, severe pelvic pain, fever with feeling very unwell, or heavy bleeding after a cervical procedure. Contact the treating clinic sooner for worsening pain or foul-smelling discharge after biopsy or treatment.

Ask whether the result came from screening or biopsy, which CIN grade was found, whether the entire affected area was seen, and what follow-up interval is planned. If treatment is proposed, ask how much tissue may be removed, what recovery involves, and how your pregnancy plans affect the options.
