Cardiac MRI scan: what to expect

The images can help a clinical team assess the heart’s size, pumping movement, muscle pattern, blood flow, and areas of inflammation or scarring. Unlike an X-ray, MRI does not use ionising radiation. The scanner is a short tunnel; you lie still on a padded table while signals are recorded. The machine makes loud knocking sounds, so headphones or ear protection are normally offered.

Some cardiac MRI examinations use a gadolinium-based contrast agent through a vein. Contrast can make blood flow or changes in heart muscle easier to distinguish. Tell the imaging team about kidney disease, previous reactions to contrast, implanted devices, metal fragments, or possible pregnancy before the appointment. A cardiac MRI is an imaging test; it does not by itself confirm cancer or another diagnosis.

Follow the appointment instructions about eating, drinking, caffeine, and regular medicines. Wear clothing without loose metal and remove jewellery, cards, and electronic items. A technologist checks your safety questionnaire, places monitoring stickers on your chest, and may ask you to hold your breath briefly during image capture. If contrast is planned, a small cannula is inserted. The team can communicate with you throughout the scan, and you can signal if you feel distressed.

Afterward, most people can return to their usual activities unless their care team gives different advice. Drink according to your normal medical instructions and follow any specific guidance after contrast. If you have an implanted cardiac device, the centre must confirm that the device and the scan conditions are suitable before scanning.

Contact the imaging service promptly if you develop a delayed rash, facial swelling, breathing difficulty, or faintness after contrast. Seek urgent medical help for severe chest pressure, marked breathlessness, collapse, or a new fast or irregular heartbeat, whether these symptoms occur before or after the appointment. A new symptom needs clinical assessment; the scan result should be interpreted alongside your history, examination, and other tests.

Ask why the cardiac MRI is being recommended, whether contrast or a medicine to improve image quality is planned, and whether you should change any medicines or food and drink beforehand. You can also ask when the report will be discussed and what further test would be considered if the images are unclear.
