Calculating a due date with irregular periods

The first day of the last menstrual period still provides a starting point when it is known, but it assumes a cycle pattern and ovulation timing that may not apply. A due date remains an estimate rather than a prediction of the exact day labour will begin.

Ovulation can occur at different times from one irregular cycle to another. Bleeding may also be mistaken for a menstrual period, making a calculation based only on the last bleed less precise.

Anovulation often presents with irregular or absent periods. Polycystic ovary syndrome commonly presents with irregular cycles because ovulation may be infrequent. These patterns explain dating uncertainty; they do not determine the health or location of a pregnancy.

During an early scan, the clinician measures the developing pregnancy and compares that measurement with the menstrual estimate. The maternity team then records the estimate used to plan later care.

Write down the first day and pattern of your last several bleeds, any positive ovulation test, and the date of the first positive pregnancy test. Mention recent hormonal contraception, breastfeeding, fertility treatment, or a recent pregnancy because these can affect cycle interpretation.

Book antenatal care after a positive pregnancy test. The clinician can arrange an appropriate scan, review medicines and supplements, and explain which estimated date will be used in your records. Avoid repeatedly changing the date with consumer apps once the clinical estimate has been set.

Contact a healthcare service promptly for bleeding, persistent pelvic pain, or severe vomiting in pregnancy. Seek emergency care for intense or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, heavy bleeding, or feeling seriously unwell. These symptoms need assessment regardless of the estimated due date.

Ask how the current estimate was chosen, whether an ultrasound is needed, and which date should be used for future appointments. If scan and menstrual dates differ, ask what that means for scheduling care and whether any follow-up is planned.
