What to know about C4-C6 ACDF

The surgeon reaches the spine from the front of the neck. Spacers or graft material maintain the prepared disc spaces, and fixation may be added to hold the operated levels in position as bone joins them.

The name C4-C6 describes a construct spanning three vertebrae and two disc spaces. Surgery may be proposed when compression at both spaces corresponds with arm symptoms, spinal cord findings, or functional problems and non-operative care is unsuitable or has not helped enough.

Because two motion segments are fused, the procedure differs from a single-level ACDF in its extent. The expected benefit, implant choice, recovery plan, and effect on neck movement should therefore be discussed for your particular anatomy.

Review all prescription medicines, non-prescription products, allergies, nicotine use, dental concerns, and prior reactions to anesthesia with the care team. Arrange help with transport and household tasks during the early recovery period.

Use pain medicine only as directed, keep the incision clean according to discharge advice, and respect restrictions on lifting and driving. Gentle walking helps restore routine without loading the neck heavily. Do not add neck exercises until the surgical or physiotherapy team confirms they are appropriate.

Report a wound that becomes hotter, redder, more painful, or starts draining. The team also needs to know about fever, persistent vomiting, increasing swallowing difficulty, or arm symptoms that are clearly worsening.

Breathing difficulty, sudden neck swelling, new weakness in an arm or leg, inability to walk safely, or new loss of bladder or bowel control warrants urgent medical assessment. Do not drive yourself if weakness or severe symptoms make travel unsafe.

Ask why both C4-C5 and C5-C6 need treatment and whether each level matches a specific examination or imaging finding. Request a clear explanation of whether the primary aim is relief of nerve-root pressure, spinal-cord pressure, or both.

Confirm the proposed spacer, graft, plate, and screw arrangement; the follow-up imaging plan; and the circumstances in which a collar is used. It is also useful to discuss alternatives and which symptoms may be less likely to change.
