What to know about C4-C5 ACDF surgery

ACDF stands for anterior cervical discectomy and fusion. At C4-C5, the operation treats one motion segment. The surgeon may place a spacer or bone graft in the emptied disc space and may add a plate and screws for stability while fusion develops.

C4 and C5 are neighboring vertebrae in the middle of the cervical spine. Disc wear, a disc protrusion, or bony narrowing at this level can crowd a nerve root or the spinal cord. The intended benefit depends on matching the pressure seen on imaging with the person's examination and symptoms.

Fusion stops movement at the operated segment. Other neck levels continue to move, although stiffness and swallowing discomfort can occur early after an approach through the front of the neck.

Before surgery, give the team a complete medicine list and disclose smoking or nicotine use, allergies, previous neck operations, and swallowing problems. Follow the hospital's instructions about fasting and which medicines to pause or continue.

Afterward, follow the surgeon's limits for lifting, driving, wound care, and collar use. Walking is commonly encouraged in short, comfortable periods. Increase activity gradually, and attend follow-up so the team can assess the wound, symptoms, and fusion progress.

Contact the surgical team promptly for increasing wound redness, drainage, fever, worsening arm pain, new numbness, or swallowing difficulty that is getting worse rather than settling.

Seek urgent medical care for trouble breathing, rapidly expanding neck swelling, new or worsening arm or leg weakness, loss of balance, or loss of bladder or bowel control. These warning signs require assessment without waiting for a routine appointment.

Ask which structure is compressed, how that finding explains your symptoms, and what improvement the operation is intended to achieve. Clarify whether a PEEK spinal cage, bone graft, plate, or screws are planned and why.

Also discuss alternatives, expected activity restrictions, signs of a wound or swallowing problem, and how recovery will be monitored. Ask what symptoms may persist even if decompression and fusion go as planned.
