The short answer

Brown discharge often contains blood that has taken longer to leave the uterus or vagina. An ectopic pregnancy develops outside the main cavity of the uterus and may present with vaginal spotting, pelvic or abdominal pain, or no clear symptoms at first. Assessment is needed to distinguish it from other causes of early-pregnancy bleeding.

What the spotting may mean

Spotting is lighter than a menstrual flow and may appear only on underwear or toilet tissue. Brown colour suggests older blood, not a particular diagnosis. Ectopic pregnancy can present with spotting, especially when it occurs alongside pain low in the abdomen or pelvis, pain focused on one side, shoulder-tip discomfort, dizziness, or weakness.

A clinician may consider your pregnancy dates, symptoms, examination findings, blood tests, and a transvaginal ultrasound. Sometimes repeat assessment is necessary because a very early pregnancy may not yet be visible clearly.

What to do now

Contact your maternity service or clinician promptly if you are pregnant and have unexplained spotting. Note when it began, whether the amount is changing, and whether you have pain, dizziness, shoulder discomfort, fever, or unusual discharge. Use a pad rather than a tampon so you can monitor the bleeding, and avoid driving yourself if you feel faint or unwell.

When to seek urgent care

Go to an emergency department now for severe or worsening abdominal or pelvic pain, shoulder-tip pain, fainting, marked dizziness, weakness, pale or clammy skin, heavy bleeding, or a sudden feeling that you are very unwell. These warning signs can accompany internal bleeding and need urgent assessment. Do not wait for the spotting to turn red or become heavier.
