Why bleeding patterns can change

During perimenopause, ovulation becomes less predictable and hormone levels fluctuate. The uterine lining may therefore build up and shed at unexpected times, causing spotting between periods, shorter or longer cycles, missed periods, or a heavier flow. Perimenopause can present with irregular bleeding, but it is not the only possible explanation. Pregnancy-related bleeding, fibroids, polyps, infection, thyroid problems, medicines, and changes in the cervix or uterine lining can also alter bleeding. A clinician can decide which possibilities fit your age, symptoms, medical history, and pregnancy risk.

When to seek urgent care

Seek urgent medical care if bleeding is very heavy, you feel faint or weak, you have severe or worsening pelvic pain, or you may be pregnant and develop bleeding with one-sided pain, shoulder-tip pain, dizziness, or collapse. Prompt assessment is also appropriate for fever with pelvic pain or foul-smelling discharge. Bleeding after menopause, meaning after periods have stopped completely for a sustained interval, should always be medically assessed rather than assumed to be part of perimenopause.
