Bleeding mid-cycle while taking the pill

Light spotting is often manageable, but bleeding that is heavy, persistent, painful, or accompanied by pregnancy symptoms needs assessment. Keep taking the pill according to its instructions unless a clinician tells you to change it.

The combined pill keeps the womb lining thin. Small areas can shed at unexpected times, producing pink, red, or brown spotting. This is more likely during adjustment to a new pill or after tablets are taken late. Vomiting, severe diarrhoea, and medicines that reduce pill effectiveness can also disrupt hormone exposure.

Bleeding can have another cause, including pregnancy, an infection, a cervical change, or a condition affecting the womb or ovaries. The pattern alone cannot identify the cause.

Check your pill packet for instructions about missed tablets, sickness, and additional contraception. Record when the bleeding began, its amount, pain, discharge, missed pills, and any new medicine or supplement. If pregnancy is possible, take a pregnancy test at the time advised on its instructions.

Arrange a routine appointment if spotting continues, returns repeatedly, begins after a previously stable pattern, or is bothering you. A clinician may review how you take the pill, check interactions, offer infection testing, or discuss a different formulation.

Seek urgent medical care for very heavy bleeding, fainting, severe or worsening pelvic pain, shoulder-tip pain, marked weakness, or bleeding with a positive pregnancy test. These features require prompt assessment, including for pregnancy complications.

Contact a clinician soon if you have fever, unusual or unpleasant-smelling discharge, pain during sex, bleeding after sex, or symptoms of anaemia such as breathlessness and unusual fatigue.

Ask whether your pill-taking pattern or another medicine could explain the bleeding, whether you need a pregnancy or infection test, and whether an examination is appropriate. You can also ask how long to monitor the pattern and what contraceptive alternatives fit your health history.
