Bleeding during an ectopic pregnancy

Bleeding may be light, dark, intermittent, or mistaken for a period. Pelvic or abdominal pain, especially on one side, can occur alongside it. A suspected or diagnosed ectopic pregnancy requires prompt clinical care because rupture can cause dangerous internal bleeding.

In an ectopic pregnancy, the pregnancy develops outside the main cavity of the womb, most often in a fallopian tube. Hormonal changes can make the womb lining shed, causing vaginal bleeding. Stretching or bleeding around the pregnancy can cause pelvic pain, while a ruptured tube can release blood into the abdomen.

Symptoms cannot confirm the pregnancy location. Clinicians combine the history with pregnancy-hormone blood tests and imaging, often a transvaginal ultrasound. Sometimes repeat blood tests or scans are needed when the location is not yet visible and the person remains stable.

If ectopic pregnancy has been diagnosed or is being investigated, contact the treating unit immediately when bleeding or pain begins or changes. Follow the monitoring plan and keep scheduled blood tests and scans. Use a pad to observe the flow, and have your treatment details and emergency contact number available.

Management depends on symptoms, scan findings, blood-test trends, pregnancy location, and overall health. Options may include close monitoring, medicine, or surgery such as salpingectomy. New symptoms require reassessment even during monitoring or after medicine, because the risk is not judged from vaginal bleeding alone.

Call local emergency services or go to an emergency department for sudden or severe abdominal pain, shoulder-tip pain, fainting, collapse, pronounced dizziness, weakness, confusion, breathlessness, clammy skin, or rapidly worsening symptoms. These can signal internal bleeding and need immediate treatment.

Do not wait for heavy vaginal bleeding: a ruptured ectopic pregnancy may bleed mainly inside the abdomen. Avoid driving if you feel dizzy, weak, or faint. Tell emergency staff that you are pregnant or may be pregnant and that ectopic pregnancy is suspected or confirmed.
