Bleeding after intercourse and endometriosis

Bleeding may come from the vagina or cervix rather than endometriosis itself. Even a small amount that recurs should be discussed with a clinician so the source can be identified.

Endometriosis commonly involves pelvic pain, painful periods and pain during or after sex. Post-coital bleeding can instead follow dryness or friction, changes on the cervix, a cervical polyp, inflammation or an infection. These possibilities cannot be separated by symptoms alone.

Note whether blood appears only after penetration or also between periods, and whether you have unusual discharge, pelvic pressure, fever or urinary symptoms. This pattern helps a clinician decide which examination or tests are appropriate.

Pause intercourse if it is painful or triggers further bleeding. If dryness may be contributing, use a water-based lubricant and avoid scented washes or products that irritate the vulva or vagina.

Arrange a medical appointment for recurrent bleeding, bleeding without an obvious minor injury, or any new change in your usual endometriosis symptoms. Assessment may include a pregnancy test when relevant, an examination of the vagina and cervix, infection testing, or imaging based on your history.

Seek urgent medical care for heavy or continuing bleeding, severe or rapidly worsening pelvic pain, fainting, marked dizziness, weakness, fever, or foul-smelling discharge.

Urgent assessment is also needed if pregnancy is possible and bleeding occurs with one-sided pelvic pain, shoulder-tip pain, dizziness or fainting. These warning signs matter regardless of whether you have endometriosis.

Ask where the bleeding appears to be coming from, whether cervical screening is due, and whether infection testing or a pregnancy test is suitable. You can also ask whether your pain pattern fits endometriosis activity or suggests another pelvic condition.

Bring details of your menstrual cycle, medicines and hormonal treatments, and explain whether penetration causes pain at the entrance or deeper in the pelvis. Mention any fertility plans because they can affect the choice of symptom treatment.
