A natural part, crown whorl or bright overhead light may make scalp visible without a true loss of density. A new smooth circle can be a presentation of alopecia areata. Short broken hairs with scale or itch can occur with scalp ringworm. Gradual miniaturisation may instead make a crown or hairline look increasingly sparse. The appearance alone cannot confirm which process is responsible.

Choose one well-lit place and photograph the area with dry, unstyled hair from a fixed angle. Check periodically rather than several times a day. Record whether hairs are shedding from the root or snapping, and note scale, itch, pain, recent illness, new medicines and styling tension. Handle the area gently and avoid unprescribed medicated creams, since some can alter the scalp's appearance before examination.

Make a clinical appointment if serial photographs show expansion, the patch is clearly smooth and bare, or additional areas develop. Prompt assessment is appropriate for marked tenderness, spreading redness, drainage, fever, thick crust or a boggy swelling. A shiny surface, absent follicle openings or loss involving brows and lashes also merits early dermatology review because these findings can change the diagnostic approach.

Show your comparison photographs and ask whether hairs are missing, miniaturised or broken. Find out whether scale should be tested, whether the follicle openings look preserved, what diagnosis is being considered, and which specific change would mean returning sooner. Mention similar hair loss, autoimmune illness or fungal infection in close contacts when relevant.
