Do not label significant breathing difficulty as anxiety without checking for asthma warning signs. Use prescribed reliever medicine as directed, sit upright and seek urgent help if breathing does not improve or any severe feature appears.
Where the symptoms overlap and differ
Both episodes can bring rapid breathing, a racing heart, chest discomfort, dizziness and fear. Asthma often presents with wheeze, cough, difficulty breathing out and symptoms after triggers such as smoke, dust, infection or exercise. Panic may include tingling around the mouth or fingers, trembling, a sense of unreality and rapidly escalating fear.
These clues are not definitive. Fear can follow genuine airway narrowing, while fast breathing during panic can intensify chest tightness. Response to prescribed reliever medicine and objective breathing measurements, when available in your plan, can help a clinician interpret the episode.
What to do during an episode
Stop activity and sit upright in a quieter place away from any obvious trigger. Follow the reliever instructions in your personal asthma action plan. Loosen restrictive clothing and ask someone to stay with you while you monitor whether speaking and breathing become easier.
If severe asthma features are absent and panic sensations remain, slow the breathing gently by making the out-breath comfortable and unforced. Avoid repeated deep gasps, which can worsen light-headedness. Do not breathe into a paper bag because reduced oxygen can be dangerous when the cause is uncertain.
When breathing difficulty is an emergency
Call emergency services if you cannot speak in full sentences, lips look blue or grey, you become confused or exhausted, breathing is severely difficult, chest movement is markedly laboured, or prescribed reliever treatment is not helping. Sudden collapse or severe chest pain also requires immediate help.
Book a clinical review when episodes keep returning, the distinction remains unclear, night symptoms occur or fear leads you to avoid normal activity. Review can address asthma control, inhaler technique and a separate plan for panic symptoms.
