Asthma and wheezing

Asthma-related wheeze often appears with cough, chest tightness, or breathlessness and may vary after exercise, cold air, respiratory infections, or allergen exposure. The pattern matters more than the sound alone.

During asthma symptoms, the airway lining becomes inflamed, muscles around the airways tighten, and mucus may add further obstruction. Air moving through the narrowed passages can create a musical or whistling noise, often most noticeable while breathing out.

Wheezing can also occur with viral airway illness, COPD, bronchiolitis in young children, or a severe allergic reaction. New wheeze therefore needs context, especially if asthma has never been diagnosed.

Pause exertion, sit upright, and move away from an obvious irritant. If you have an asthma action plan, use the prescribed reliever and follow its instructions for reassessment. A spacer can improve delivery when it forms part of your prescribed setup.

Book a review when wheezing is new, keeps returning, disturbs sleep, restricts normal activity, or is not responding as it usually does. Bring your inhalers so the clinician can check technique, and describe recent infections, allergy symptoms, smoking or vaping exposure, and medicine changes.

Seek emergency help if breathing is very difficult or worsening quickly, speech is limited by breathlessness, the lips look blue or grey, you feel faint or confused, or the prescribed reliever gives little relief. Wheeze accompanied by swelling of the lips or tongue, widespread hives, or sudden collapse may be anaphylaxis and requires immediate emergency treatment.

Ask what is driving the wheeze, whether breathing tests would clarify the diagnosis, which inhaler is for quick relief, and which medicine controls airway inflammation. Confirm what changes should trigger a routine appointment, same-day assessment, or an emergency call.
