ALEX allergy test: purpose, results, and limitations

ALEX is a multiplex blood test that examines responses to a broad panel of allergen extracts and individual molecular components from foods, pollens, mites, moulds, insects, and animals. A clinician should select and interpret it against the person's symptoms and exposure history.

Specific IgE means the immune system recognises a substance. Clinical allergy means exposure also causes compatible symptoms. Someone may be sensitised without becoming unwell, so avoiding every item marked on a report can create unnecessary dietary or lifestyle restrictions.

Component results can sometimes help distinguish a response to a major allergen from cross-reactivity between related proteins. They may refine risk discussions, but they do not predict the severity of a future reaction with certainty. Negative findings also need context if the suspected trigger was absent from the panel or the history remains convincing.

Before the blood draw, write down the suspected trigger, amount or type of exposure, time to symptoms, organs affected, treatment used, and whether the event has happened again. Bring ingredient labels or photographs when available. Antihistamines generally do not suppress specific IgE in a blood sample, but disclose all medicines and follow the laboratory's instructions.

Review the complete report with an allergy-trained clinician. Do not reintroduce a food that previously caused a serious reaction, and do not remove many foods solely because of low-level sensitisation. A supervised challenge or a more targeted test may be considered when history and blood findings do not agree.

Call emergency services for difficulty breathing, throat tightness, tongue swelling, collapse, severe dizziness, or rapidly worsening symptoms affecting more than one body system after an exposure. If an adrenaline auto-injector has been prescribed, use it immediately according to the person's action plan and then seek emergency help.

Do not wait for an ALEX result during an acute reaction. Blood testing is used for later assessment and does not replace emergency treatment for anaphylaxis.

Ask which findings match the reaction history, which are likely cross-reactions, whether any avoidance is necessary, and whether a supervised food challenge or targeted testing would clarify the diagnosis. Confirm an emergency action plan and auto-injector technique if there is a risk of anaphylaxis.
