What to know about ADH surgical excision

The clinician usually numbs the area, cuts out the lesion, and closes the wound with stitches when appropriate. The laboratory report identifies the tissue and assesses whether abnormal cells extend to the specimen edges. That result helps determine whether the excision is complete or whether another procedure should be considered.

Excision can be used for a suspicious growth, a confirmed skin cancer, or a bothersome benign lesion when removal is appropriate. The planned margin and closure depend on the diagnosis, size, depth, body site and skin tension. A standard excision differs from Mohs surgery: standard excision is sent for routine laboratory processing, while Mohs surgery examines mapped tissue edges during the procedure.

A scar is expected because the skin is cut and repaired. Its final appearance is influenced by wound position, healing, sun exposure, infection and individual scar tendency. Numbness, tightness, bruising and mild tenderness can occur near a fresh wound.

Give the surgical team a complete medication list, including blood thinners, supplements and medicines that affect immunity. Do not stop prescribed medication unless the clinician who manages it gives specific instructions. Mention allergies to local anaesthetic, adhesives or dressings, and explain any previous problems with bleeding or raised scars.

Afterward, follow the written dressing and washing instructions. Keep the wound protected, avoid pulling or stretching across the stitches, and attend the arranged visit for results or suture removal. Use only the pain relief and wound products recommended for you. Once the surface has healed, sun protection can help limit colour change around the scar.

Seek prompt advice if bleeding continues despite firm pressure, the wound opens, pain is becoming stronger rather than settling, or redness is spreading. Increasing warmth, swelling, pus, an unpleasant smell or fever may indicate infection and should be assessed. Emergency care is appropriate for heavy uncontrolled bleeding, fainting, breathing difficulty or a severe allergic reaction.

Contact the treating clinician when the laboratory result has not arrived within the expected timeframe. A report showing involved margins does not automatically describe the next step; the diagnosis, site and remaining tissue all shape whether further excision, Mohs surgery or observation is discussed.

Ask what diagnosis the procedure is intended to confirm or treat, how wide and deep the removal is expected to be, and how the wound will be closed. It is also useful to ask when washing and exercise can resume, who will communicate the pathology result, what the margin wording means, and what options are available if more treatment is needed.
