Why gout becomes acutely painful

Options may include a non-steroidal anti-inflammatory medicine, colchicine, or a corticosteroid. These are not interchangeable for everyone: kidney disease, stomach problems, blood-thinning treatment, pregnancy, infection risk, and medicine interactions influence the choice. Do not start, stop, or change prescribed urate-lowering medicine without instructions from the prescriber.

When the joint needs prompt review

Seek same-day assessment for a first episode of a hot, swollen joint, uncertainty about the diagnosis, or symptoms that are worsening despite treatment. Fever, chills, feeling very unwell, a wound near the joint, recent joint surgery or injection, or reduced immunity raises concern for joint infection, which requires urgent care.

Emergency help is appropriate if illness is accompanied by confusion, fainting, breathing difficulty, or rapidly spreading redness. After the acute inflammation settles, follow-up can address urate measurement, kidney health, medicine review, and whether prevention treatment is suitable.
