A wheezy cough: causes and warning signs

The wheeze is often more noticeable while breathing out. A rattling sound from mucus or a whistle from the nose may be mistaken for chest wheeze, and listening to the lungs can help distinguish them.

Asthma often presents with episodes of cough, wheeze, chest tightness or breathlessness, sometimes after exercise, cold air or allergens. Acute bronchitis and the common cold can cause cough with temporary airway irritation. Bronchiolitis may present with cough and wheeze in babies and young children.

Allergic rhinitis can drive cough through nasal congestion and throat irritation. Acid reflux may provoke cough after meals or when lying down. Bronchiectasis commonly presents with a persistent productive cough and repeated chest infections, and may include wheezing. The pattern guides assessment but does not confirm a diagnosis.

Avoid smoke, vaping aerosols, strong fragrances and any recognised trigger. Drink enough fluid to avoid dehydration and use prescribed inhalers according to your personal plan; check that your spacer and inhaler technique are correct.

Record whether the cough is dry or brings up mucus, when it is worse, and whether fever, breathlessness, reflux, nasal symptoms or chest tightness occur. Arrange a clinical review for a new wheezy cough, recurring episodes or symptoms that disturb sleep or normal activity.

Call emergency services if breathing is severely difficult, lips become blue or grey, the person is confused or collapses, speech is limited by breathlessness, or the face or throat swells. A baby with pauses in breathing, marked chest pulling, poor feeding or unusual drowsiness also needs urgent assessment.

Obtain prompt medical advice for coughing up blood, chest pain, dehydration, persistent fever, worsening wheeze or prescribed reliever medicine that is not working as expected.
