What malaise can feel like
Feeling generally unwell is a broad sense of being off-colour, with reduced comfort, energy or normal functioning.
You may feel achy, weak, feverish, unsettled or unable to pinpoint what is wrong. It can occur with a sore throat, cough, nausea, headache, appetite change or fatigue, but sometimes starts before clearer symptoms appear. The change from your usual state is useful information.
Signs needing urgent assessment
Seek urgent medical help for confusion, blue or grey lips, severe breathing difficulty, chest pain, a stiff neck with fever, a new widespread purple rash, persistent vomiting, severe abdominal pain, or rapidly worsening illness. Get prompt advice when there are signs of dehydration such as very little urine, inability to keep fluids down, or marked dizziness on standing.
Possible reasons for feeling unwell
Malaise may be part of a viral illness, another infection, inflammation, a medicine effect, disrupted sleep or a longer-term condition. The conditions below are alphabetical rather than a likelihood ranking. Common cold, COVID-19, glandular fever, hepatitis A, influenza and sepsis can all present with a general feeling of illness, alongside their other features.
How the cause is checked
A clinician will ask about the timeline, close contacts, travel, food and drink exposures where relevant, medicines, and accompanying symptoms. They may check temperature, pulse, breathing, hydration and the throat, chest, abdomen or skin. Swabs, blood tests, urine testing or imaging may be considered when the examination and symptoms suggest a particular cause. Share any condition that affects immunity or any recent procedure.
Treatment follows the underlying problem
Rest, fluids and simple symptom relief may be appropriate for a mild self-limiting illness, while other causes need specific treatment and follow-up. Antibiotics do not treat viral illnesses, so the decision to use them depends on the suspected infection. Follow the treatment plan you are given, and seek reassessment if symptoms worsen, new red flags appear, or you are not recovering as expected.
Reducing exposure and protecting recovery
Hand hygiene, ventilation, staying away from others when acutely unwell, safe food and water practices, and recommended vaccinations can reduce some infectious illnesses. Give yourself time to recover before returning to demanding activity. If you care for someone who is ill, avoid sharing drinks or utensils and clean frequently touched surfaces.
Details to mention at a consultation
Report fever, chills, rash, cough, sore throat, diarrhoea, vomiting, urinary symptoms, yellowing of the skin or eyes, severe headache, neck stiffness, breathlessness or pain. This information helps separate a mild illness from a problem that needs faster investigation.
Following the course of illness
A vague unwell feeling may resolve as a minor illness settles, or it may develop into a clearer symptom pattern. Checking in when the direction is worsening is more useful than trying to identify the cause alone. Sepsis is a serious infection-related emergency and needs immediate assessment when severe illness or rapid deterioration occurs.

Questions people ask
Is feeling generally unwell the same as fatigue?
They can overlap. Fatigue centres on low energy, whereas malaise is a broader sense of illness or discomfort that may include fatigue.
Can I monitor mild symptoms at home?
You may be able to monitor mild, improving symptoms while resting and drinking fluids, but seek medical advice if symptoms worsen, you cannot stay hydrated, or any urgent warning sign develops.
