Understanding your Total Testosterone results
Testosterone is the main male sex hormone (androgen). It is made mainly in the testes, and in smaller amounts by the adrenal glands in both sexes. The total testosterone test measures all testosterone in the blood: the small free fraction, the part loosely bound to albumin and the part tightly bound to SHBG (sex hormone-binding globulin). Levels follow a daily rhythm and are highest in the morning, which is why samples are usually taken then.

What a high Total Testosterone result can mean
In men, a result above the reference range is uncommon in someone who is not using testosterone treatment or anabolic steroids. In women, raised testosterone is most often associated with polycystic ovary syndrome (PCOS), which can also cause irregular periods, excess hair growth (hirsutism) and acne. Less common causes involve the adrenal glands.
What a low Total Testosterone result can mean
In men, low testosterone together with symptoms is called hypogonadism. Symptoms linked to it include reduced libido, tiredness, loss of muscle and low mood. When the testes are the cause, LH and FSH are high (primary hypogonadism). When the pituitary is not stimulating the testes, LH and FSH are low or normal (secondary hypogonadism). In women, a low result is rarely significant on its own.
How a doctor reads the result
A doctor will usually interpret testosterone alongside symptoms, the time the sample was taken, weight, medicines and other hormones. A single low reading is commonly repeated on another morning before any conclusion is drawn. Discuss the result with your doctor.
Follow-up a doctor may arrange
A doctor may repeat the test in the morning and add LH, FSH and prolactin to see where the problem lies. In women with a raised result, a doctor may arrange a pelvic ultrasound and other hormone tests. Any hormone treatment is prescribed and monitored by a specialist.
