DDA’s AED 7,500 freelance permit shows the real cost of UAE clinic contractors
Freelance permits are only the first cost. Clinics must price in insurance, visas, licensing checks and patient billing risk.
How Zavis verifies this coverage
Editorial standards, source rules, methodology, and review provenance are public.
Dubai freelance permits can start at AED 7,500 a year, but clinics using independent clinicians, therapists or health consultants should budget for visa, insurance, Emirates ID and licensing checks before treating the day rate as the full cost.
The IndexBox item on UAE freelancing costs matters most to clinic CFOs, COOs and HR teams. The immediate issue is margin. A contractor who looks cheaper than an employee can become more expensive once onboarding time, regulator checks, insurance eligibility and claim acceptance are added.
Dubai costs clinics must price before contracting
The clearest public Dubai benchmark is from the Dubai Development Authority (DDA), which lists a freelancer permit fee of AED 7,500 per annum, plus AED 10 Knowledge Dirham and AED 10 Innovation Dirham per transaction. Some commercial packages in the market quote lower entry prices, but those packages may exclude residence visa, medical fitness, Emirates ID, establishment card, workspace access, health insurance and renewal service fees.
For a Dubai clinic, that distinction changes the procurement question. The day rate is one line. The compliance file is another. A contractor providing regulated care still needs the relevant healthcare professional licence and permission to practise in the correct facility setting. In Dubai, that means the Dubai Health Authority (DHA) for most private sector facilities, with Dubai Healthcare City Authority rules applying inside DHCC.
- Permit cost: check the free zone or mainland authority’s official fee schedule, starting with DDA where applicable.
- Visa cost: confirm whether the quote includes entry permit, medical fitness test, Emirates ID and residency stamping steps.
- Insurance cost: verify whether the policy meets the emirate’s residency and payer-network rules.
- Clinical licence: confirm DHA eligibility, active licence status and facility attachment before the first appointment.
Insurance is a cost floor, not a clinical access guarantee
Health insurance is the hidden cost that most directly affects patients. The Ministry of Human Resources and Emiratisation (MoHRE) said its basic health insurance package costs AED 320 per year and became a requirement for issuing or renewing residency permits from 1 January 2025 in the relevant emirates. That gives employers and sponsors a price floor. It does not tell a clinic whether a freelancer’s policy gives access to its network, whether specialist visits require prior approval, or whether claims will be accepted for services delivered by an external contractor.
Dubai’s private clinics should involve revenue cycle teams before using freelancers in billable patient pathways. A locum doctor, dietitian, physiotherapist or mental health professional may be licensed, but the payer file still needs the correct provider identifiers, facility link and pre-authorization route. Insurers including Sukoon sell health cover in the UAE market, while Daman is central in Abu Dhabi and Thiqa applies to eligible UAE nationals under Abu Dhabi’s government-funded programme. Each payer’s network and billing rules should be checked directly, rather than inferred from the freelancer’s visa status.
Abu Dhabi and the northern emirates have separate checks
In Abu Dhabi and Al Ain, the healthcare regulator is the Department of Health Abu Dhabi (DOH). Abu Dhabi’s Law No. 23 of 2005 applies the health insurance scheme to non-UAE national residents and their families in the emirate. A freelancer based in Dubai may still need Abu Dhabi-specific licensing and payer checks before working in a DOH-regulated clinic.
In Sharjah, Ajman, Ras Al Khaimah, Fujairah and Umm Al Quwain, clinics should check Ministry of Health and Prevention (MOHAP) licensing and the relevant free zone or economic department fee schedule. MOHAP’s public medical facilities directory is a starting point for facility verification, but professional licensing status should be confirmed through the applicable regulator’s live portal.
The practical action is to treat freelance engagement as a three-file exercise: commercial contract, immigration and insurance file, and healthcare regulatory file. CFOs should price the full annual cost before approving contractor rates. COOs should add licence, facility attachment and payer credentialing checks to onboarding. HR teams should keep renewal dates for permits, insurance and professional licences in the same tracker.
Patients see only whether an appointment is available and whether insurance pays. Clinics carry the risk behind that transaction. Before adding independent clinicians to a rota, operators should verify licensed clinics and providers through the UAE Open Healthcare Directory and the relevant DHA, DOH or MOHAP register.
