Women’s Period Cycle: When It Is Irregular

Short answer
An irregular period pattern can result from changing ovulation, hormones, contraception, stress, or illness, so its timing and associated symptoms guide assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What an irregular cycle can mean
Cycles may vary in length, bleeding may be lighter or heavier, or a period may be absent. Puberty, breastfeeding, perimenopause, major stress, intense exercise, weight change, and some medicines can alter the pattern. When ovulation does not happen regularly, bleeding can become unpredictable; Anovulation is one possible explanation. Polycystic Ovary Syndrome can also affect ovulation, often alongside acne or increased facial hair.
A clinician may consider pregnancy, thyroid problems, anaemia, and other hormonal causes. Premature Ovarian Insufficiency is another condition to discuss when periods stop earlier than expected, especially with hot flushes or fertility concerns.
What to do now
Record the first day of each bleed, how long it lasts, flow changes, pain, spotting, medicines, contraception, and related symptoms. Take a pregnancy test when relevant. Arrange a routine appointment if the pattern persists, changes after previously regular cycles, or affects fertility or daily life. The assessment may include a history, examination, pregnancy testing, blood tests, or imaging depending on the pattern.
Treatment depends on the cause and your goals. The Combined Pill may regulate bleeding for some people, while Metformin for PCOS may be considered in selected situations; neither should be started without individual advice.
When to seek care
Seek urgent care for very heavy bleeding with dizziness, faintness, shortness of breath, or severe pain. Get prompt assessment for bleeding during pregnancy, bleeding after menopause, or sudden severe pelvic pain. A missed period with pain or unusual bleeding needs timely medical advice.
Questions for your doctor
What could explain my pattern and symptoms? Do I need a pregnancy test, blood tests, or imaging? Could my contraception or medicine be involved? How might treatment affect bleeding, fertility, or other health goals?

Questions people ask
Can stress change periods?
Yes. Significant stress can affect hormone signalling and ovulation, which may change cycle timing or bleeding.
Should a missed period be checked?
If pregnancy is possible, test first. Persistent absence, repeated irregularity, or concerning symptoms merits clinical assessment.
