Why does sinusitis cause earache?

The Eustachian tube connects the middle ear to the back of the nose. It normally opens to equalise pressure. During sinus inflammation or a cold, swelling and mucus near its opening can interfere with that process, giving a feeling of fullness, popping, muffled hearing, or an ache. Earache is a common presentation of Sinusitis, although ear pain can also come from an ear infection, jaw problems, teeth, or throat inflammation.

Pain that is sharp, worsening, or confined to one ear deserves particular attention. A clinician can look in the ear and nose to distinguish pressure-related discomfort from another problem.

Rest, fluids, and saline nasal spray or rinses may help with congestion if suitable for you. Swallowing, yawning, or chewing may briefly relieve pressure for some people. Use only gentle pressure changes; do not forcefully blow your nose or try to pop an ear when it is painful. Avoid inserting cotton buds, oils, or other objects into the ear. They do not clear middle-ear pressure and can injure the ear canal.

Get urgent help for swelling or redness behind the ear, severe headache, stiff neck, new weakness of the face, severe dizziness, sudden hearing loss, or fluid or blood from the ear. Seek a medical review for persistent earache, reduced hearing, fever with worsening illness, or pain that is not settling as the nasal symptoms improve.

Ask whether the ear pain appears to be pressure from congestion or an ear condition needing separate treatment. Mention hearing changes, discharge, recent swimming, dental pain, and whether one side is affected, as these details can guide the examination.
