Why do ovarian cysts cause pelvic pain?

A cyst can make the ovary heavier or enlarge it, causing aching, pressure, or pain with movement. Pain may be central or stronger on one side, and it can spread into the lower back. A cyst that leaks fluid may irritate nearby tissue; twisting can reduce blood flow and cause abrupt, severe pain.

Pelvic pain also occurs with menstrual cramps, endometriosis, infection, urinary problems, bowel conditions, and pregnancy-related emergencies. Its presence does not establish one diagnosis. The timing, severity, examination, and pregnancy possibility all affect the next step.

Record when the pain began, its location and intensity, its relation to periods or activity, and any bleeding, discharge, bowel, urinary, or pregnancy-related symptoms. Make an appointment when pain persists, returns, or changes. A clinician may recommend examination and imaging to assess the ovary and distinguish a cyst from other causes.

Seek urgent care for sudden severe pelvic pain, fainting, shoulder-tip pain, heavy bleeding, fever, repeated vomiting, or pain with a possible pregnancy. Prompt assessment matters because rupture, twisting, infection, or an ectopic pregnancy can require immediate treatment.
