Why do ovarian cysts cause ovulation pain?

Ovulation pain is often felt on one side of the lower abdomen and may occur near the middle of the menstrual cycle. A cyst can add pressure to the ovary, and fluid or minor bleeding from a cyst can irritate the lining of the pelvis. Pain may be brief, aching, or sharper, and it can sometimes be felt in the lower back. The timing and sensation alone cannot identify the type of cyst or confirm its cause.

A clinician may consider your cycle pattern, examination findings, and an ultrasound. Many ovarian cysts change over time, so follow-up may be suggested to see whether the cyst resolves or remains.

Track when the pain starts, which side is affected, how long it lasts, and whether bleeding, nausea, or bowel and bladder symptoms occur. Rest, gentle movement, and a warm compress may be soothing. Ask a pharmacist or clinician which pain medicine is suitable for you, especially if you are pregnant, have stomach or kidney problems, or take medicines that affect bleeding.

Arrange a medical review if the pain keeps returning, interferes with daily activities, or occurs with a known cyst. Assessment can help distinguish cyst-related discomfort from other pelvic causes.

Seek urgent medical help for sudden severe pelvic pain, pain with repeated vomiting, faintness, marked weakness, fever, or heavy vaginal bleeding. These symptoms can occur when a cyst ruptures or twists the ovary, although other urgent conditions can cause them too. Do not wait for the next cycle if symptoms are intense or rapidly worsening.
