Short answer

Kyphosis can cause upper back pain by changing spinal alignment and making muscles work harder to support the head and torso.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Spine region
Kyphosis most often refers to an increased forward curve of the upper back.
Pain pattern
Muscle fatigue, stiffness and aching may increase after sustained sitting or standing.
Assessment
A physical examination helps guide treatment and decide whether further tests are needed.

Why kyphosis can hurt

Upper Back Pain is a common presentation of Kyphosis. The amount of discomfort can vary with the cause of the curve, its flexibility, posture, activity and the tissues involved.

What is happening in the back

The thoracic spine naturally curves forward, but a more pronounced kyphotic curve can shift the shoulders and upper body forward. Muscles along the spine, between the shoulder blades and around the neck may stay active for longer to keep you upright. That sustained effort can produce aching, tightness or fatigue.

The altered shape can also change how force moves through the small spinal joints and ribs. Stiff joints may make turning or extending the upper back uncomfortable. In some people, the curve is flexible and posture-related; in others, it is linked with vertebral changes, ageing, injury or another condition. An examination helps distinguish these possibilities.

What may help

A clinician or physiotherapist can assess your posture, spinal movement, muscle strength and pain pattern. Treatment may include exercises that build endurance in the back and shoulder muscles, improve chest and upper-back mobility, and support safer movement habits. A gradual programme is usually easier to maintain than forcing the spine into a straighter position.

Keep moving within comfortable limits, change position regularly during desk work, and avoid repeatedly lifting or reaching through pain. Heat may ease muscle tightness for some people. Do not start forceful stretching or spinal manipulation until the cause of the curve and your symptoms have been assessed.

When to seek care

Arrange a clinical assessment if pain persists, returns often, limits sleep or daily activities, or comes with a visibly changing curve or increasing stiffness. Seek urgent medical help for new weakness, numbness, trouble walking, loss of bladder or bowel control, severe pain after an injury, or breathing difficulty.

These warning signs can indicate nerve, spinal or chest problems that need prompt assessment. Upper-back pain can have several causes, so the symptom alone cannot establish kyphosis.

Questions for your doctor or physiotherapist

You could ask: What may be contributing to my spinal curve and pain? Is my curve flexible? Which movements or exercises are suitable for me? Do I need imaging or a review by another specialist? What changes should prompt an earlier appointment?

Questions people ask

Can kyphosis cause pain between the shoulder blades?

Yes. Muscle overload and stiffness around the thoracic spine can cause discomfort between the shoulder blades, although other causes are possible.

Will correcting my posture cure kyphosis pain?

Improving movement habits may reduce strain, but the right approach depends on whether the curve is flexible, structural or related to another condition.

Should I exercise with kyphosis and upper back pain?

Gentle, appropriately selected movement is often useful, but a clinician or physiotherapist should tailor exercises when pain is persistent or the curve is changing.
