What upper back pain can feel like
Upper back pain, also called thoracic back pain or pain between the shoulder blades, may feel aching, tight, burning, sharp or tender. It can stay in one area or spread across the shoulder blades, ribs or the base of the neck. Turning, reaching, lifting, sitting for a long time or taking a deep breath may change the sensation. Muscle-related pain may feel worse when the area is pressed or when a particular movement loads it. Describe when it started, what movement changes it, and whether numbness, weakness, fever, chest symptoms or recent injury are present.
Upper back pain can arise from muscles, posture, spinal alignment, irritated tissues or an injury, so assessment focuses on its pattern and possible cause.
Is upper back pain urgent?
Arrange prompt medical assessment after a significant fall or blow, especially if the pain is severe or movement is restricted. Seek urgent help for upper back pain with chest pressure, trouble breathing, fainting, sudden weakness, new loss of bladder or bowel control, or numbness spreading into the limbs. Pain with fever or unexplained feeling of being very unwell also needs timely assessment. Without these warning features, book an appointment if symptoms persist, repeatedly return, disturb sleep, or limit work and daily activity.
Possible causes
The cause cannot be ranked from the order below; these are different explanations that a clinician may consider. Kyphosis can change the curve of the upper spine and increase muscular loading. Muscle Strain may follow lifting, exercise or a sudden movement. Myofascial Pain Syndrome involves sensitive muscle and connective-tissue areas that can refer pain. Text Neck describes discomfort associated with sustained forward-head and screen posture. Vertebral Compression Fracture can cause sudden or persistent spinal pain, particularly after an injury or when bone is fragile.
How it is assessed
A clinician asks about onset, activities, injury, posture, sleep, general health and symptoms in the arms or chest. Examination may include observing spinal alignment, shoulder movement and breathing, then checking tenderness, strength, sensation and reflexes. Imaging is not needed for every presentation; it may be considered when the history or examination suggests a fracture, structural change or another condition requiring a closer look. Bring a list of medicines and note activities that ease or aggravate the pain.
Treatment by possible cause
Management is matched to the finding. A muscle strain may benefit from a gradual return to movement, activity modification and a clinician-guided exercise plan. Posture-related symptoms may improve with workstation changes, movement breaks and strengthening of the upper back and neck. Physiotherapy can address mobility, motor control and load tolerance; Myofascial Release may be considered for selected soft-tissue symptoms. Kyphosis or a suspected compression fracture needs an assessment-led plan rather than forceful self-treatment. Cupping Therapy may be discussed as an adjunct when appropriate, but it does not identify the cause.
Reducing the chance of recurrence
Build activity gradually and vary tasks that keep the shoulders or neck in one position. Keep screens near eye level, support the forearms when working, and take brief movement breaks before stiffness builds. Strength and mobility exercises should be progressed according to symptoms and guidance. Use lifting technique that keeps the load close and avoid sudden increases in training volume. Symptoms that may occur alongside it
Upper back pain may occur with stiffness, reduced shoulder movement, neck discomfort, muscle spasm or tenderness around the shoulder blades. Chest discomfort or breathlessness should not be assumed to come from the back; urgent assessment is appropriate when these symptoms are sudden, severe or unfamiliar.

Track the pattern for a few days: location, triggers, relieving positions, sleep impact and any associated symptoms. A primary-care clinician, orthopedist or physiotherapist can help decide whether examination, imaging or rehabilitation is appropriate. Persistent pain that limits normal movement deserves a clear working diagnosis and a follow-up plan.
