Why does gluteal tendinopathy cause hip pain?

Short answer
Hip Pain is a common presentation of Gluteal Tendinopathy because sensitive gluteal tendons are strained during movement and compressed at the outer hip.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical location
- The outer hip, sometimes extending to the buttock or upper thigh.
- Common loads
- Stairs, running, standing on one leg and rising from a chair.
- Recovery approach
- Reduce aggravating load, then progress strengthening gradually.
Why the hip becomes sore
The gluteus medius and gluteus minimus tendons help keep the pelvis level as you walk and stand on one leg. Repeated loading, a sudden change in exercise or reduced muscle support can make these tendons more sensitive. The pain is often felt over the bony outer hip, although it can spread into the upper outer thigh or buttock.
Hip Pain may increase with stairs, running, getting out of a chair, or lying on the affected side. These tasks either demand more tendon force or press the tendon against nearby bone.
How to calm and rebuild it
Modify the activities that provoke symptoms and avoid long periods lying on the painful side. Keep moving at a level that does not produce a marked flare, and ask a physiotherapist for a progressive strengthening plan. The exercises may begin with low-load holds before advancing to controlled hip and leg movements.
Comfortable footwear and pacing can help some people manage walking. Medication or other treatments should be discussed with a clinician, especially if you have stomach, kidney or heart concerns.
When to seek care
Seek an assessment if hip pain limits walking, sleep, work or stairs, or if it follows a fall. Get urgent help for inability to bear weight, sudden major weakness, numbness with bladder or bowel changes, fever, or a hot and swollen joint.
Questions for your doctor
Is the pain centred over the gluteal tendons or could the joint or back be involved? Which exercises can improve load tolerance? How should I adjust stairs, running and side-lying? When should persistent pain be reassessed?

Questions people ask
Why does lying on my side hurt?
Side-lying can compress the sensitive gluteal tendons against the outer hip and increase local pressure.
Can gluteal tendinopathy cause pain while walking?
Yes. Each step asks the gluteal tendons to stabilise the pelvis, and hills or longer walks may increase that demand.
