Where the pain comes from

The gluteal tendons attach the buttock muscles to the outer side of the hip. They help stabilise the pelvis while you walk, climb stairs, stand on one leg or change direction. When their capacity is exceeded by a sudden increase in activity or repeated strain, the tendon can become sensitive to load.

Pain may sit deep in the buttock or near the outer hip. Sitting for a long time, crossing the legs, lying on the painful side or taking large steps can compress the tendon and make symptoms more noticeable.

Practical steps

Temporarily reduce activities that sharply increase pain, then build them back gradually. Avoid prolonged positions that compress the outer hip, such as sitting with the knees crossed or lying directly on the sore side. A physiotherapist can select progressive strengthening for the gluteal muscles and adjust the load as symptoms respond.

Comfortable walking is often preferable to complete rest, but a flare after activity is a reason to review the amount, speed or terrain rather than push harder.

Arrange an assessment if buttock pain persists, disrupts sleep, changes your walking or limits stairs and daily tasks. Seek urgent care for severe pain after a fall, inability to bear weight, new leg weakness or numbness, loss of bladder or bowel control, or fever with a hot, swollen area.
